Appointments can be made by phone or through our online
Patient Portal. Please arrive fifteen minutes early for your appointment and have your
insurance card, photo ID and list of medications ready. An interpreter
can be provided if needed. Any copayment, past-due balance or pre-pay
must be paid prior to seeing the doctor. Payment is due when services
are rendered. We welcome all forms of health insurance.
Newborns up to ten weeks may accompany you into the exam room. Unless the
child is a patient, all children must remain in the waiting room with an adult.
Please make refill requests at least one week in advance and have your
pharmacy’s number available when you call us. You may leave your
request on our automated phone service or through our
Patient Portal. New medications or refills will not be filled after-hours, weekends or holidays.
