We know life moves fast. That’s why we’re proud to offer MyChart, a secure, online patient portal that helps you quickly access your health information, request appointments, view lab results, message your doctors and much more – anytime, anywhere.

MyChart is for everyone.

If you have visited a CaroMont Health hospital or doctor’s office, you are eligible for a MyChart account. You can even manage your child’s health information or request proxy access for other family members (such as elderly parents) with permission. MyChart is completely free and easy to use.

Signing up is easy.

You can sign up at your doctor’s office, use the activation code provided in your visit statement or request an activation code online. The code will enable you to create your completely confidential MyChart account.

    With online scheduling, you don’t have to call the office to schedule an appointment. Just log in and pick the time and date that work best for your busy schedule.

Please do not use MyChart to send messages about urgent medical issues. For emergencies, contact your doctor’s office or call 911.
