Patient Information

Patients are seen by appointment only. When you call to make an appointment, please describe your medical problem to ensure you are schedule appropriately and conveniently. If you needed to cancel or reschedule your appointment, please give us 24 hours advanced notice. In the event of an emergency, call our office and explain in as much detail as possible the problems you are having. Our after hours answering service will page the provider on call and you will be contacted immediately with further instructions.

If you are a new patient, please arrive 20 minutes prior to your scheduled appointment and bring your filled out welcome packet available on our website, list of current medications, insurance card, photo id and a form of payment. Out of concern for our obstetrical patients, we ask that you do not bring children to our office.

Please be prepared to pay for services when they are rendered, including copays, coinsurances and deductibles. As a courtesy to our patients, we will gladly file your insurance following your delivery, surgery and/or special procedures. We do accept most forms of health insurance however, we request that all patients present pertinent insurance information prior to being seen.

Some minor routine lab tests are done in our office and results are available immediately. More complex lab tests are done at an outside laboratory and may take a few days for results to be returned. We will notify you of your results by phone or through our online Patient Portal.
