Patient Information
Patients will be required to make a Payment when services are rendered.
If you are unable to PA - Cy in full, or your co-payment is not available
at the time of your visit, your appointment may be rescheduled. Patients
under an HMO or PPO contract require you to pay your co-payment at the
time of each visit.
Please bring a copy of your insurance card, a valid photo ID, and a list
of all current medications to your appointment.
Refills can be requested by contacting your pharmacy. The pharmacy will
then contact us for authorization or additional information. Controlled
substances and other medications cannot be filled on weekends, evenings,
or holidays.
Medical records or information will only be released in accordance with
HIPA - CA regulations. You must provide a written release of authorization
prior to your health information being released to other Parties.
Our Patient portal offers and easy and secure way to access health information,
view and schedule appointments, view lab results and ask questions. Our
staff are also available to assist with any additional questions you may
have. Additionally, they will be happy to assist you in obtaining access
to our My Chart electronic
Patient Portal.
