Patient Information
Patients should arrive 15 minutes prior to their scheduled appointment
and bring a list of their current medications, photo ID, insurance card
and a form of payment. All appointment requests and test results are available
via phone or by logging onto your
MyChart Patient Portal.
Patients should contact their pharmacy for routine prescription refills.
The pharmacy will contact us if they need additional information. Certain
medications, such as controlled substances, will not be filled on weekends,
holidays or evenings.
Payment is expected when services are rendered. If you are under an HMO
or PPO, your copayment is due at the time of each visit. Please have your
insurance card available each time you visit the office, before you see
the doctor and before you have lab tests performed. Read your insurance
information carefully. Call your insurance company or our office if you
have any questions.
Your medical records are confidential. We will not release any medical
information unless we have written authorization from you.
