Patients should arrive 15 minutes prior to their scheduled appointment
and bring a list of their current medications, photo ID, insurance card

For routine refills, please contact your pharmacy. They will contact us
with needed information. Certain medications, such as controlled substances,
will not be filled on weekends, holiday or evening.
Payment is expected when services are rendered. If you are under an HMO
or PPO, contract requires you pay your copayment at the time of each visit.
Please have your insurance card available each time you visit the office
and before you see the doctor or have any lab tests performed. Read your
insurance information carefully. Call your insurance company or our office
if you have any questions.
Your medical records are confidential. We will not release any medical
information unless we have written authorization from you.
