All patients are seen by appointment only. Please arrive 15 minutes prior
to your scheduled visit if you are an established patients and 30 minutes
early if you are new to our practice. Please bring MRI films and all medications
to your appointment. Your appointment may be rescheduled if these requests
are not met.
We understand appointments may need to be canceled or rescheduled. If you
need to cancel your appointment, please contact our office immediately
and with at least 24 hours notice.
The purpose of your evaluation is to obtain a comprehensive understand
of your condition. We ask that you complete all paperwork and provide
the following items at your visit so our staff may better serve you.
- Current photo ID and insurance, Medicare or Medicaid card
- Current list of your medications and dosages, or bring your medication bottles
- MRI films
- Name, address and phone number of your previous healthcare providers
- A form of payment
You must bring your insurance card to each visit. Regardless of your insurance
status, you are ultimately responsible for the payment of services. If
you need to make payment arrangements, please call our Billing Office
Your medical records are confidential. We will not release any medical
information unless we have written authorization from you.
Test results will be communicated by phone or via our online Patient Portal.
Patients can also request follow-up appoints through the portal as well.
