Patient Information
Appointments are made by a referral from the hospital or a primary care
provider. Remember to arrive fifteen minutes early for your appointment
and bring a list of your current medications with you. New patients are
asked to arrive thirty minutes early to complete their new-patient paperwork.
Interpreters for any language are available for patients. Same-day urgent
appointments can be made as well.
Payment is expected when services are rendered. If you are under an HMO
or PPO, contracts require you pay your copayment at the time of each visit.
With each visit, you will receive a statement that itemizes all charges
and summarizes your account. Please have your insurance cards available
at each visit, before you see the doctor or have any lab tests performed.
Call your insurance with any questions you may have regarding coverage.
Routine refills may be completed through your pharmacy. Certain medications,
such as controlled substances, will not be filled on weekends, holidays
or evenings. Test results will be made via phone or the
Patient Portal. Any follow-up tests should be completed prior to the follow-up visit.
We will not release any of your medical information unless we have written
consent from you.
