Patient Information
Appointments can be made by phone or through our online
Patient Portal. Once patients enter the lobby, our receptionist will assist in the check-in
process. Please arrive 15 minutes prior to your scheduled appointment
and bring any current medications, insurance card, a photo ID and any
copay or coinsurance information. Follow-up appointments will be scheduled
as needed after each appointment is completed. Test results can be accessed
through the
Patient Portal, mail or telephone. If needed, an interpreter from Fluent Language can
be provided.
For routine prescription refills, contact your pharmacy. Certain medications,
such as controlled substances, will not be filled on weekend, holidays
or after normal clinic hours.
Payment is expected when services are rendered. If you are under a HMO
or PPO, contracts require you pay your copayment at the time of each visit.
Please have your insurance cards available each time you visit the office,
before you see the doctor or have any lab tests performed.
Your medical records are confidential. We will not release any medical
information unless we have written authorization from you.
