Patient Information
Same-day appointments are available and can be made during regular office
hours. Please bring a photo ID, your insurance card and any current medications
to your appointment. Payment is expected when services are rendered and
if you are under an HMO or PPO, contracts require you to pay your copayment
at the time of each visit.
Prescription refills can be made by contacting your pharmacy. Controlled
substances and other medications cannot be filled on weekends, evenings
or holidays. Medical records or information will not be released unless
you have provided written authorization.
Appointment requests can be made through the
Patient Portal or by phone. Please remember to arrive fifteen minutes prior to your appointment.
If a patient requires an interpreter will be provided for the patient
if needed. Follow-ups and tests results will be provided to the patient
after the visit.
