Patient Information
We are currently accepting new patients, and appointments can be made by
phone or through the
Patient Portal. Same day appointments are usually available. Patients should bring their
medications, insurance card, copayment and any coinsurance information
and arrive fifteen minutes early for your appointment. A bilingual Spanish-speaking
interpreter is available on site, as well as Dr. Vacalis, who is our Spanish-speaking
physician.
Payment is expected at the time of service. We participate with most major
insurance plans, as well as self-pay patients.
We offer 24-hour access to your health information through our secure
Patient Portal. You can view your upcoming appointments, send secure messages to your
provider, review lab results, update insurance and demographic information
and manage your billing statements online. Patients will receive test
results and follow-up appointment requests by phone or through the
MyChart.
For routine refills, please contact your pharmacy. They will contact us
with information we need. Certain medications, such as controlled substances,
will not be filled on weekends, holidays or evenings.
