Sponsorship Request Submission Process

1. Complete Sponsorship Request Form and submit to Amy Pendergast, Sponsorship Coordinator at katie.wayne@caromonthealth.org, 704-834-2250 (facsimile), or via mail at 2525 Court Drive, Gastonia, North Carolina 28054
2. Attach documentation showing proof of 501(c)(3).
3. Attach a completed, current IRS Form W-9.
4. Attach any additional information you feel would be helpful to us in making a decision.

Reminder:  Sponsorship Request Forms received outside the month of February will not be reviewed.  Your request should cover events occurring between July 1 of the year in which you are applying, and June 30^th of the following year.
