The CaroMont Health Foundation is proud to offer financial assistance to individuals entering the health care field.

Applicant Criteria:

Preferred but not limited to:

• Residency in Gaston County, North Carolina
• Scholastic Achievement
• Financial Need
• Applicants with a relationship to CaroMont Health as an employee or volunteer
• Applicants with a relative(s) employed or volunteering with CaroMont Health
• Applicant’s interest in pursuing a degree in a health care profession
• Applicant’s acceptance into an accredited college or university

Required Information:

All information must be received by the Foundation office on or before the stated deadline in order to be eligible for consideration.

1. College and high school official transcripts be sent to:
   CaroMont Health Foundation
   RE: Shovelin Scholarship
   2525 Court Drive
   Gastonia, NC 28054
2. Proof of acceptance and/or enrollment into college or university program
3. Two (2) personal letters of recommendation
4. Volunteer/Community Involvement/ School Activities: (see Page #3)
5. Why and how would this scholarship impact the advancement of your healthcare career? (see Page #3)
6. Applications must be completed in full.

Application deadline is 5:00 PM on January 3, 2012
Foundation Committee will meet with final 5 Applicants on February 22, 2012
