Patient Information

Patients will be required to make a payment when services are rendered. If you are unable to pay in full, or your copayment is not available at the time of your appointment, the visit may be rescheduled. Patients under an HMO or PPO contracts require you to pay your copayment at the time of each visit. Please bring a copy of your insurance card, a photo ID and a list of current medications to your appointment.

Refills can be made by contacting your pharmacy. The pharmacy will then contact us for any further information. Controlled substances and other medications cannot be filled on weekends, evenings, or holidays.

Medical records or information will not be released unless you have provided written authorization.

Our Patient Portal offers an easy and secure way to access health information, view appointments, view lab results and ask questions. Our staff is also available for any additional questions you may have.
