Patient Information

Upon entering the building, patients will make a right into the Carolinas Plastic Surgery Center waiting room. Please remember to bring your insurance card, copayment, any coinsurance information and a list of current medications. New patients are asked to arrive twenty minutes prior to their scheduled appointment to complete paperwork. If you are an established patient and have not been seen in over a year, please plan to arrive early to update any information. An interpreter can be provided if needed.

Payment is expected when services are rendered. If you are under an HMO or PPO, contracts require you to pay your copayment at the time of each visit. With each visit, you will receive a statement that itemizes all charges and summarizes your account.

For routine refills, please contact your pharmacy. They will contact us with information we need. Certain medications, such as controlled substances, will not be filled on weekends, holidays or evenings.

Your medical records are confidential. We will not release any medical information unless we have written authorization from you. Test results can be distributed through the Patient Portal, but our staff prefers to speak with the patient to discuss results or findings.
