Patient Information

Patients should arrive 15 minutes prior to their scheduled appointment time and bring their insurance card, a list of current medications, photo ID and a form of payment.

Appointments can be made Monday - Friday during regular office hours, 8:30 a.m. to 5:00 p.m. When you call, provide a brief description of your medical problem to allow us to schedule accordingly. We may charge for missed appointments not canceled 24 hours in advance.

For routine refills, please contact your pharmacy. They will contact us with the information we need. Certain medications, such as controlled substances, will not be filled on evenings, weekends or holidays.

Payment is expected when services are rendered. If you are under an HMO or PPO, contracts require that you pay your copayment at each visit. Costs will vary based on your individual appointment. You will receive a statement for each visit that itemizes all charges and summarizes your account.

Please have your insurance cards available each time you visit the office, before you see a provider or have any lab test performed. Read your insurance information carefully. Call your insurance company or our office if you have any questions.

Your medical records are confidential. We will not release any medical information unless we have written authorization from you. Please make sure that all personal information is accurate and up-to-date.

Call 911 or proceed to the Emergency Department. Call us if your need is less urgent and we will send a message to your physician or expedite an office or hospital evaluation. A nursing service is available to assist with after-hours calls.

CaroMont Medical Group uses an online patient portal allowing patients access to view health records and payments, view and request appointments, retrieve test results and more.
