Patient Information

Please arrive 15 minutes early for your appointment and have a photo ID, your current insurance, current medication list and copayment ready. We ask that new patients arrive thirty minutes early to complete new-patient paperwork. An interpreter will be provided for the patient if needed. Follow-ups and tests results will be provided to the patient after the visit.

Payment is expected when services are rendered. If you are under an HMO or PPO, contracts require you to pay your copayment at the time of each visit. Prescription refills can be made by contacting your pharmacy. Medications cannot be filled on weekends, evenings or holidays. Medical records or information will not be released unless you have provided written authorization.
