For identification purposes and to make your payment, please have your most recent statement and bank account or credit card information available. To make a payment, enter the patient’s Account Number as it appears on the statement and the patient’s Date of Birth.

Payment in full is expected unless prior arrangements have been made. Please allow 48 hours for payment processing.
